    Understanding sciatica: causes, symptoms and physiotherapy in Vauxhall

    Sciatica is one of the most common reasons people across Vauxhall, SW8 and the wider Lambeth and South London area come to see us. It's not a diagnosis in itself — it's a description of pain that travels along the sciatic nerve, which runs from the lower back through the buttock and down each leg. When that nerve is irritated or compressed, the pain can be unmistakable: sharp, electric, and often felt well below the site of the actual problem.

    The good news is that most cases of sciatica respond well to conservative treatment, and a thorough physiotherapy assessment is usually the right first step.

    What causes sciatica?

    Sciatica is a symptom, so the question is always what's irritating the nerve? The common causes include:

    • Herniated or bulging discs. A disc in the lumbar spine pressing on a nerve root is the most frequent cause. Our guide to herniated discs goes into more detail.
    • Spinal stenosis. Narrowing of the spinal canal, often age-related, that reduces the space available for the nerves.
    • Piriformis syndrome. The piriformis muscle in the buttock can sometimes irritate the sciatic nerve as it passes nearby.
    • Degenerative changes. As discs and joints in the spine age, they can lose height and stiffness, increasing load on the nerves.
    • Spondylolisthesis. A vertebra that has shifted forward on the one below it, sometimes compressing nerve roots.

    A careful clinical assessment matters because the cause shapes the treatment. Sciatica from a disc behaves differently from sciatica driven by a tight piriformis, and the rehab plan reflects that.

    Symptoms to look out for

    People describe sciatica differently, but common features include:

    • A sharp or shooting pain radiating from the lower back into the buttock, the back of the thigh, and sometimes all the way to the foot.
    • Numbness, tingling, or pins-and-needles along the same path.
    • Weakness in the leg or foot — for example, difficulty pushing off when walking.
    • Pain that worsens with prolonged sitting, standing, or specific positions, and may ease with gentle movement.

    Sciatica is usually one-sided. If you have any sudden loss of bowel or bladder control, numbness around the saddle area, or rapidly worsening leg weakness, that is a medical emergency — go straight to A&E rather than waiting for a physio appointment.

    How physiotherapy helps

    The aim of physiotherapy for sciatica is twofold: settle the nerve irritation, and address whatever is causing it so the problem doesn't keep coming back. At our Vauxhall clinic we typically combine:

    • A detailed assessment. Where exactly is the pain? What movements aggravate it? What's happening at the spine, hips, and core? This guides everything that follows.
    • Manual therapy. Hands-on techniques to ease muscle guarding and improve segmental movement where it's restricted.
    • Targeted exercise. A graded programme — usually starting with gentle, nerve-friendly movement and progressing to strengthening for the trunk, glutes, and hips.

    When to come in

    If your sciatica has been there longer than a couple of weeks, is interfering with sleep or work, or is recurring, it's worth a proper assessment.
